Mechanism of Action

Meloxicam powder, classified as one of the NSAID, is a selective cyclooxygenase-2 (COX-2) inhibitor. COX enzymes (COX-1 & COX-2) produce various compounds in the body, including prostaglandins and thromboxane. It works by blocking the action of cyclooxygenase (COX)-1 and COX-2, thereby preventing the synthesis of prostaglandins. Prostaglandins raise body temperature and make nerve endings more sensitive to pain transmission. It has a preferential effect on COX-2 over COX-1.

COX-2 is primarily produced at sites of inflammation and is responsible for the signs and symptoms of inflammation at the site of injury. Inhibition of COX-2 by meloxicam at sites of inflammation results in decreased prostaglandin production. Since prostaglandins cause swelling of injured tissue and increase pain sensitivity, meloxicam reduces the production of prostaglandins, resulting in less swelling and less sensitivity to pain, thereby managing pain.

While COX-1 is mainly located in the gastrointestinal tract and is more protective of the mucosa of the gastrointestinal tract, preventing the development of gastric ulcers, secondary to NSAID use. Meloxicam has a milder effect on the GI mucosa and is better tolerated than traditional NSAIDs such as diclofenac because meloxicam is less inhibitory to COX-1 than COX-2,

Meloxicam is unique among NSAIDs in the market because of its selective activity against COX-2. The COX-2 enzyme is not present in the stomach like COX-1. This means that meloxicam may not cause as many side effects in the stomach compared to NSAIDs that are not selective for COX enzymes.

What are the Uses of Meloxicam?

There are two main uses of Meloxicam as follows.

1) Rheumatoid arthritis in adults

Rheumatoid arthritis (RA) is an autoimmune disease. It means that the body's immune system attacks its cells, causing pain and inflammation. Meloxicam is approved for treating rheumatoid arthritis in adults and children.

A 12-week randomized placebo-controlled trial in 894 RA patients showed that meloxicam reduced joint swelling, tenderness, pain, and overall symptoms. Meloxicam did not cause significantly more gastric side effects than placebo.

In a 6-month, double-blind trial of 379 RA patients, meloxicam reduced joint pain and swelling better than another typical NSAID, naproxen.

2) Osteoarthritis

Osteoarthritis (OA) is a common type of arthritis that affects the articular cartilage that protects the ends of bones. Numerous clinical trials have shown that meloxicam can reduce the pain and inflammation related to this condition.

For example, a 12-week randomized placebo-controlled trial including 774 patients with OA found that meloxicam reduced symptoms of pain and stiffness. In this study, there was no difference in the number of gastric side effects between meloxicam and placebo.

Meloxicam also reduced pain in a 6-month, multicenter, randomized trial of 336 patients with OA. Meloxicam caused fewer side effects than another NSAID, diclofenac.

In another randomized trial of 229 patients with lumbar spine OA, meloxicam improved pain and movement. Meloxicam was again better tolerated than diclofenac.

Side Effects

-Stomach-related side effects such as indigestion, heartburn, and bleeding. Constipation, diarrhea, gas, nausea, vomiting, or bleeding may also occur;

Older adults, people taking other medications which can affect the stomach or those who drink more than three drinks a day may be more likely to experience side effects, including stomach-related side effects.

-Increased risk of stroke or heart attack;

Patients with pre-existing cardiovascular disease and higher doses may be at higher risk. It may not be unfit for those with a history of kidney disease, stomach ulcers, or other gastrointestinal problems, as well as those with pre-existing cardiovascular disease or repeated heart attacks. Don't give to patients who have recently had coronary artery bypass surgery.

-Not a substitute for aspirin for the prevention of cardiovascular events;

-May interact with other drugs such as warfarin, SSRIs, ACE inhibitors, and diuretics.
