What Is the Side Effects of Imatinib

The bcr-abl kinase of CML was inhibited by imatinib.

The most chronic side effects include nausea, vomiting, diarrhea, headache, leg pain/cramps, fluid retention, visual impairment, itching of skin rashes; decreased resistance to infection, bruises or bleeding, loss of appetite; weight gain, blood cells (neutropenia, thrombocytopenia, anemia) and edema. Because it depends on CYP3A4 for metabolism, it is not recommended in people who use potent CYP3A4 inhibitors such as ketoconazole, clarithromycin, chloramphenicol, ritonavir, and nefazodone. Similarly, it is CYP3A4, CYP2D6, and CYP2C9 inhibitors, so simultaneous treatment with any of these enzyme substrates may increase the plasma concentration of the drug. Because imatinib is mainly metabolized by the liver enzyme CYP3A4, the substances that affect the enzyme's activity will change the drug's plasma concentration. Ketoconazole is an example of a drug that increases imatinib activity by blocking CYP3A4 and thus increases side effects. The same is true of itraconazole, clarithromycin, grapefruit juice, etc.

Conversely, CYP3A4 inducers such as rifampicin and St. John's wort reduce the activity of the drug and thus risk treatment failure. Imatinib also works as an inhibitor of CYP3A4, 2C9, and 2D6, increasing plasma concentrations of many other drugs, such as simvastatin, cyclosporine, pimozide, warfarin, metoprolol, and possibly paracetamol. The drug also reduces plasma levels of levothyroxine through unknown mechanisms.

Like other immunosuppressants, the use of live vaccines is taboo because the microbes in the vaccine multiply and infect patients. Inactivated and toxoid vaccines do not have this risk but are probably ineffective under imatinib treatment.

What Is the Mechanism of Action of Imatinib

Imatinib is a 2-phenylamino pyrimidine derivative, which can be used as a specific inhibitor of many tyrosine kinases. It occupies the active site of TK, resulting in a decline in activity.

There are a great number of TK enzymes in the body, including insulin receptors. Imatinib is specific to the TK domain of abl (Abelson proto-oncogene), c-kit, and PDGF-R (platelet-derived growth factor receptor).

In chronic myeloid leukemia, the Philadelphia chromosome causes a fusion protein called bcr-abl that fuses abl with bcr (breakpoint cluster region). Imatinib reduces bcr-abl activity because it is a constitutively active tyrosine kinase.

Each tyrosine kinase active site has an ATP binding site. The enzyme activity catalyzed by tyrosine kinase is the transfer of terminal phosphate from ATP to the tyrosine residue on its substrate, a process called protein tyrosine phosphorylation. Imatinib binds to the ATP binding site of bcr-abl and locks it in a closed or self-inhibitory conformation, thus semi-competitively inhibiting the enzyme activity of the protein. This explains why many BCR-ABL mutations lead to resistance to imatinib by shifting its balance to an open or active conformation.

Imatinib is highly selective to bcr-abl, although it inhibits other targets mentioned above (c-kit and PDGF-R), ABL2 (ARG) and DDR1 tyrosine kinase and NQO2--, an oxidoreductase. Imatinib also inhibits abl proteins in good cells. Still, these cells usually have extra redundant tyrosine kinases. That allows them to continue to function even when abl tyrosine kinases are inhibited. However, some tumor cells are dependent on bcr-abl. Inhibition of bcr-abl tyrosine kinase also stimulates it to enter the nucleus, where it cannot perform any normal anti-apoptotic function, resulting in tumor cell death.

Imatinib is absorbed quickly after oral administration and has high bioavailability: 98% of the oral dose enters the bloodstream. The metabolism of imatinib happens in the liver and is mediated by some isozymes of the cytochrome P450 system, CYP3A4 included and, to a lesser extent, CYP2D6, CYP1A2, CYP2C9, and CYP2C19.
