Background Information
Setipiprant powder was first developed by Actelion as a prostaglandin D2 receptor anticonjugation agent. The simple understanding is that setipiprant can prevent the binding of prostaglandin D2 to its receptors. According to the research, prostaglandin D2 (referred to as PGD2) is related to allergy and inflammation in the body, so in the beginning setipiprant was originally developed to treat some symptoms of allergy and inflammation, especially to treat asthma.
Although setipiprant has shown good drug tolerance and efficacy in clinical trials, it does not have much superiority compared with the existing drugs for treating asthma in the market. So lastly, the follow-up related experiments were gradually stopped.
Shortly after, the clinical trials of the drug started back again, but this time as a prostaglandin receptor antagonist – the PGD2 receptor, more specifically.
In March 2012, Professor George Cotsarelis of the University of Pennsylvania published an article in the journal Science Translational Medicine. The core content is that the researchers found that the concentration of prostaglandin D2 in the scalp of the hair loss part of patients with androgenetic alopecia is higher than that of the prostate in the non-hair loss part. The concentration of prostaglandin D2 is 3 times that of prostaglandin D2, so it is judged that prostaglandin D2 has a great relationship with hair loss. The study took mice as the object and found that high concentrations of prostaglandin D2 can indeed inhibit the growth of hair in mice, and as long as prostaglandin D2 is reduced, the hair can return to normal. The article concludes that inhibition of prostaglandin D2 may be a solution for the treatment of male pattern baldness in the future.
How does it work?
Setipiprant is an antagonist of the prostaglandin D2 receptor 2 (DPD). DPD is responsible for bonding with and responding to certain prostaglandins that are linked to various pathological responses, including those that are linked to allergies, asthma and inflammation.
Research highlighted a link between the PGD2 receptor and hair loss. This receptor is seen at high levels in the scalp of men who are diagnosed with Androgenetic Alopecia (AGA), male pattern baldness. AGA (Male-Pattern Baldness) is the most common cause of hair loss in men, though it’s also seen in women (Female Pattern Hair Loss). It is thought to be caused by a sensitivity to DHT, which is the androgen hormone that attaches to the hair follicles and leads to inflammation and irritation. The irritation caused by DHT can cause hair follicle miniaturization which over time will lead to balding.
PGD2 is produced from PGD2 synthase, an enzyme. PGD2 synthase levels are increased by DHT, with the increase in PGD2 the receptor on the scalp is triggered and this causes inflammation.
According to researchers, setipiprant steps in before PGD2 attaches to the receptors, and therefore prevents receptor activation and, as a result, hair loss.
